Job Summary

This role involves working closely with community mental health teams, GPs, and other agencies to provide care plans for patients. The successful candidate will have experience in CAMHS and a strong understanding of risk assessment and management. They will also have excellent communication skills and the ability to work effectively with patients, families, and colleagues.

Key Responsibilities:

  • Assess and manage children and young people presenting in crisis, using clinical knowledge and expertise to formulate an assessment of relapse risk of the patient.
  • Liaise with other agencies caring for patients, including community mental health teams and GPs, to provide care plans for all patients.
  • Utilise recent research findings in the advancement of best practice and in the delivery of an evidenced based service.
  • Promote general health education and specifically mental health education to the client group.
  • Ensure that individual episodes of care are delivered in a timely, effective and integrated manner.
  • Promote a recovery model that empowers patients, carers and relatives to be at the forefront of decision-making and ownership of their packages of care and treatment.
  • Undertake clinical risk assessments, based on the latest empirical evidence and in compliance with local policies and procedures.

Salary: £32,877 - £40,325 per annum (inclusive of annual leave allowance)
