Job Overview:
A challenging and rewarding opportunity exists within the Nottinghamshire Healthcare NHS Foundation Trust for a dedicated and skilled Care Coordinator (Band 6) to join our multidisciplinary team in the Early Intervention in Psychosis Service.
The successful candidate will have experience of working with individuals experiencing complex mental health presentations, including psychosis and personality disorders. They will possess excellent analytical and decision-making skills, as well as advanced interpersonal skills to effectively communicate and work with a wide range of professionals.
About the Role:
As a Care Coordinator, you will be responsible for managing a caseload of service users who have been identified as experiencing their first episode of psychosis. This will involve conducting comprehensive assessments, developing and implementing personalized care plans, and providing evidence-based interventions to support recovery.
You will also be required to work collaboratively with other healthcare professionals, including psychiatrists, psychologists, and social workers, to ensure seamless delivery of high-quality patient care.
Key Responsibilities:
- Manage a caseload of service users with first-episode psychosis
- Conduct comprehensive assessments and develop individualized care plans
- Implement evidence-based interventions to promote recovery
- Collaborate with multidisciplinary teams to ensure effective patient care
- Use digital technologies to support service delivery
Requirements:
To be considered for this role, you must hold a core professional qualification in nursing or occupational therapy and have a good understanding of the principles and ethos associated with working within an Early Intervention in Psychosis team. You should also have experience of using psychological formulations to identify intervention targets and working therapeutically with individuals experiencing psychosis.
Additionally, you should possess excellent IT skills, including Microsoft Office and digital technology platforms, and be able to prioritize workload effectively to meet deadlines.
