**Job Summary:**

This is an exciting opportunity for experienced Mental Health Nurses to support Adult Mental Health LMHTs as part of the community transformation programme.

**Key Responsibilities:**

  • Caseload management for patients living in the community
  • Acting as care coordinator, provision of initial assessments and one-to-one interventions with service users
  • Undertaking and recording comprehensive need/risk assessments, devising and implementing treatment plans for service users

**Salary:** £31,383 - £40,558 per annum (dependent on experience)

**Location:** Various locations across Nottinghamshire, including Ashfield, Bassetlaw, Broxtowe, City Central, City East, City South, Gedling, Mansfield, Newark, and Rushcliffe
