About the Role

We are seeking an experienced and skilled physiotherapist to join our team as a Senior Physiotherapist - Community Care Specialist. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality physiotherapy services to patients in their own homes.

Key Responsibilities
  • Provide comprehensive physiotherapy assessments, treatments, and advice to patients with complex needs in the community setting.
  • Manage a caseload of patients, prioritizing their care and coordinating with other healthcare professionals as needed.
  • Develop and implement individualized treatment plans, ensuring effective and efficient use of resources.
  • Collaborate with the multidisciplinary team to ensure seamless care coordination and communication.
  • Contribute to the development and implementation of service policies and procedures.
Requirements
  • Postgraduate degree in Physiotherapy or equivalent.
  • State registration with the Health and Care Professions Council.
  • Minimum 2 years of experience as a physiotherapist in a community setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
