Royal Trinity Hospice is seeking a highly skilled and motivated Lead Specialist Nurse to join our First Contact team.

Role Overview

This is an exciting opportunity to work in a dynamic environment where you will play a lead role in receiving and managing referrals made to the hospice.

Key Responsibilities:
  • Act as the first point of clinical contact for patients, identifying and responding to their specialist palliative care needs through direct liaison with the patient, carer, or other involved health and social care professionals.
  • Triage all incoming referrals, considering patients' stated symptoms to determine the appropriate level of care required to safely meet their specialist palliative care needs.
  • Utilize agreed resources and guidelines to effectively assess, prioritize, advise, schedule patient care management or refer patients when necessary to other appropriate health and social care agencies.
  • Ensure a timely, responsive, supportive, and effective service for all stakeholders involved in patient referrals.
  • Manage all aspects of the referral system to the hospice, identifying appropriateness, urgency, and level of priority.
  • Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice, ensuring care is planned to meet individualized and dynamic needs.
Required Skills and Qualifications:
  • Registered Nurse with bachelor's degree-level training in palliative/end of life care or a related subject and extensive experience working within a specialist palliative care field.
  • Advanced assessment skills.
  • Critical Thinking Skills.
  • Able to demonstrate knowledge of disease trajectories and treatment effects.
  • Advanced Communication skills.
  • Enthusiasm and a flexible attitude to working.
  • Ability to work autonomously and as part of a team.

Estimated salary: £53,645 - £60,351 per annum, pro rata, dependent on experience.
