Job Role

We are recruiting a Mental Health Community Facilitator to work with patients in mental health recovery. As a community facilitator, you will be responsible for coordinating support needs and facilitating discharge from step-down services.

Responsibilities:
  • Coordinating support for patients allocated to you, including ADL support and escorting to appointments
  • Facilitating discharge from step-down services and liaising with housing and local authorities
Requirements:
  • Experience of working in Mental Health and Social Care
  • Education or qualifications in health and social care or mental health care
  • Good knowledge of working with people in mental health crisis and their families/carers
