At the Central London Community Health Trust, we are seeking a skilled and experienced District Nurse Team Leader to join our dynamic Community Nursing Teams in Brent. This is an excellent opportunity for you to develop your leadership skills and contribute to the delivery of high-quality care in the community setting.

About the Role

This Band 7 position involves providing clinical leadership in the management of patients referred by General Practitioners, with the aim of preventing unnecessary hospital admissions. You will be responsible for coordinating care between other agencies involved in patient care, working closely with GPs to plan and deliver effective care.

The successful candidate will have experience of nurse leadership and management, as well as a sound knowledge of current issues and legislation within the NHS. You will also possess exceptional interpersonal and communication skills, with the ability to assess and deliver quality care.

Key Responsibilities
  • Manage the care of patients referred by GPs, with a focus on prevention of hospital admissions.
  • Coordinate care between agencies involved in patient care.
  • Work closely with GPs to plan and deliver effective care.
  • Provide clinical leadership and support to team members.
Required Skills and Qualifications

We are looking for candidates with:

  • A strong background in District Nursing, with at least 3 years of experience at Band 6 level.
  • District Nurse/Teaching qualification and nurse prescribing qualifications.
  • Experience of nurse leadership and management.
  • Excellent communication and interpersonal skills.
