Care Pathways Coordinator

We are seeking a highly organized and detail-focused Care Pathways Coordinator to join our team at Devon Partnership NHS Trust. This role offers an excellent opportunity to contribute to the development and implementation of effective care pathways for individuals with specialized mental health needs.
About the RoleThe successful candidate will work closely with multidisciplinary teams to design, implement, and maintain care pathways, ensuring seamless transitions between services and minimizing delays in discharge. As a Care Pathways Coordinator, you will be responsible for analyzing data, identifying trends, and making recommendations for process improvements.
Main Responsibilities- Care Pathway Development: Collaborate with teams to design, implement, and evaluate care pathways, ensuring they meet individualized needs and promote recovery.
- Data Analysis: Analyze data to identify trends, inform decision-making, and drive quality improvements.
- Process Improvement: Recommend and implement changes to care pathways and processes, ensuring they are efficient, effective, and responsive to patient needs.
- Qualifications: Registered Mental Health/Nursing or other registered health/social care professional with post-graduate qualifications or equivalent experience.
- Experience: Proven track record of working in healthcare or social care settings, with a focus on process improvement and project management.
- Knowledge and Skills: Excellent analytical and problem-solving skills, with the ability to interpret complex data and develop evidence-based recommendations.
Estimated Salary: £38,000 - £55,000 per annum
