Nurse Specialist in Community Paediatric Care

Job Overview
We are seeking a skilled and compassionate Nurse Specialist in Community Paediatric Care to join our team at Homerton University Hospital.
The successful candidate will provide high-quality nursing care to children with complex health needs in the community, working closely with families, carers, and other healthcare professionals to ensure seamless transitions of care.

Main Duties and Responsibilities
- To assess and develop individualised care plans for children with complex health needs, working collaboratively with multidisciplinary teams to ensure comprehensive support.
- To provide specialist nursing care to children in their own homes, schools, or community settings, promoting independence and empowering families to manage their child's condition effectively.
- To work closely with colleagues in acute and community settings to ensure smooth transfers of care, sharing knowledge and expertise to improve outcomes for patients.
- To contribute to the development and evaluation of evidence-based practices, supporting quality improvement initiatives and service redesign.
- To participate in education and training activities, mentoring junior staff and students to promote excellence in community paediatric care.

Requirements
- Registration as a Children's Nurse (RSCN) or equivalent qualification.
- Substantive experience in paediatric nursing, ideally in community settings.
- Ability to communicate complex information effectively, both verbally and in writing.
- Demonstrable skills in assessment, planning, implementation, and evaluation of care.
- Excellent interpersonal and teamwork skills, with the ability to build strong relationships with patients, families, and colleagues.
