About the RoleThis is an exciting opportunity to work in a dynamic and supportive team, providing care to patients in their own homes, care homes, and clinics. As a Community Nurse, you will be responsible for assessing, planning, implementing, and evaluating patient care, using your clinical expertise and judgment to make decisions that promote the best possible outcomes for patients.
Key Responsibilities- Assess patients' needs and develop individualized care plans that address their physical, emotional, and social needs.
- Provide high-quality patient care, including wound management, palliative care, and rehabilitation.
- Work collaboratively with patients, families, and healthcare professionals to deliver person-centered care.
- Participate in multidisciplinary team meetings and contribute to the development of care plans.
- Maintain accurate and up-to-date patient records, using electronic systems to record patient information.
To be successful in this role, you will need:
- A diploma or degree in nursing, with current registration with the NMC.
- Experience of working in a community setting, with a strong understanding of community health and care principles.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Able to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
