Community Health Nurse

We are seeking a highly motivated and skilled Community Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with patients, families, and healthcare professionals to provide holistic and person-centered care.
Main Responsibilities- Assess, plan, implement, and evaluate patient care, using clinical reasoning skills to develop individualized care plans.
- Conduct holistic assessments, communicating complex and sensitive information to patients and carers, and modifying care plans as needed.
- Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care is coordinated.
- Participate in multi-disciplinary/multi-agency meetings, contributing to the development of care plans and ensuring patient-centered care.
- Ensure patient-held records are completed for each visit, and SystmOne inputting is completed on a daily basis.
- Contribute to the requisitioning of supplies and equipment through electronic ordering.
- Ensure informed consent is obtained prior to initiating treatment.

Requirements- Diploma or 1st-level degree in Nursing.
- Current NMC registration.
- ENB 998/mentorship preparation or equivalent.
- Experience of working within a clinical setting.

- Ability to manage stressful situations.
- Good interpersonal skills.
- Ability to use own initiative.

- Able to manoeuvre patients using handling aids.
