Job Description

Join our team as a Senior Pharmacist, Acute, and take advantage of this unique opportunity to shape the future of integrated healthcare. You will work closely with eConsult to process patient queries and collaborate with general practice staff on medicines management.

Main Responsibilities

  1. Deliver high-quality pharmaceutical care to patients as part of multidisciplinary teams.
  2. Support ward-level pharmacy services, ensuring seamless discharge liaison and timely communication.
  3. Drive clinical pharmacy input to sub-specialties, fostering a culture of collaboration and innovation.
  4. Analyse medicine usage to inform cost-effective prescribing strategies.
  5. Raise awareness among general practice staff about the role of GP practice pharmacists and available services.
  6. Process medication eConsults, mediate medication reconciliation, and oversee medication workflow across primary care sites.
  7. Contribute to medicines optimisation initiatives, driving operational excellence and clinical impact.
  8. Design training programs to enhance general practice staff knowledge on medication-related topics.
  9. Enhance discharge processes by collecting and sharing electronic discharge information.

Salary Information
