About the Role

We are seeking a skilled and compassionate Care Worker to join our team in Norfolk. As a Rapid Responder, you will provide flexible support to customers and their families, working in a fast-paced environment to make a real difference in people's lives.

Key Responsibilities
  1. Provide person-centered care support in the community, including complex cases, and work collaboratively with the wider care team.
  2. Travel as required to deliver high-quality support, building effective relationships with stakeholders and customers.
  3. Identify and signpost patients and their families to relevant support services, preventing further admissions and promoting independence.
  4. Work in partnership with the bookings team to ensure seamless support delivery, maintaining accurate records and contributing to contractual reporting requirements.
  5. Participate in the out-of-hours on-call rota system, demonstrating flexibility and a commitment to providing exceptional care.
Requirements
  • A minimum NVQ Level 2 in health and social care or equivalent, with Level 3 desirable.
  • Experience of providing person-centered care in the community, with a strong understanding of homecare and legislative standards.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with stakeholders and customers.
  • Able to work under pressure, cope with change, and prioritize tasks effectively.
  • Willingness to travel across the region and participate in the out-of-hours on-call rota system.
