The Role

We are seeking a skilled and compassionate Care Worker to join our Crisis Response team. As a Care Worker, you will play a vital role in providing emergency support to individuals in crisis, working closely with our team to assess needs and develop personalized care plans.

Key Responsibilities

  1. Provide emergency care and support to individuals in crisis, working in partnership with our team to assess needs and develop personalized care plans.
  2. Deliver high-quality care and support in the community, including complex cases, working in partnership with our team to ensure seamless delivery of services.
  3. Travel as required to deliver person-centered support, working in partnership with our team to ensure that our customers receive the best possible care.
  4. Signpost patients and their families to relevant support services, both internal and external, to prevent further admissions and promote independence.
  5. Work in partnership with our bookings team to identify required support needs, enabling individuals to remain and return safely in their own homes.
  6. Complete contingency risk assessments, assessments, and risk assessments for new and existing customers, ensuring a 'rapid response' to customer need and maintaining high-quality support.
  7. Liaise regularly and effectively with the wider team, ensuring seamless delivery of services and promoting a culture of collaboration and respect.
  8. Work flexibly, including evenings and weekends, to meet the needs of our customers and ensure that our services are delivered to the highest standard.
  9. Maintain accurate records on in-house systems, supporting patient and family carer case management and contributing to contractual reporting requirements.

Requirements

  • A minimum NVQ Level 2 in health and social care or equivalent, with Level 3 desirable.
  • Experience of providing person-centered care in the community, with a minimum of 2 years' experience in a domiciliary setting.
  • Good IT skills, including experience using laptops and rostering systems.
  • Experience of working independently, using own initiative and problem-solving, with the ability to multitask and work productively with a range of people with different needs.
  • Strong communication and interpersonal skills, with the ability to build effective relationships with stakeholders and provide excellent customer service.
  • Able to work under pressure, cope with change and conflicting priorities, and demonstrate a willingness to travel across our region.
