Peripatetic Insulin Nurse Specialist

We are seeking a highly motivated and experienced Peripatetic Insulin Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our community nursing team, you will be responsible for providing high-quality care to patients with diabetes, working closely with GPs, Social Services, and other healthcare professionals to ensure seamless care coordination.
Key Responsibilities- Assess, plan, implement, and evaluate patient care, developing individualized care plans that meet the unique needs of each patient.
- Work collaboratively with the multidisciplinary team to facilitate early discharge from hospital and prevent unnecessary admissions.
- Provide education and support to patients and their families on insulin management and diabetes care.
- Conduct holistic assessments, communicating complex information to patients and carers, and adapting explanations to meet individual needs.
- Liaise with GPs, Social Services, and other healthcare professionals to ensure identified needs are met and care is coordinated.
- Participate in multi-disciplinary meetings, contributing to the development of care plans and ensuring patient needs are met.
- Ensure accurate and timely documentation of patient records and SystmOne inputting.
- Work flexibly to accommodate patient and service needs, demonstrating adaptability and a commitment to delivering high-quality care.
- Current NMC registration and a valid driving license.
- Diploma or degree in nursing, with experience in a clinical setting.
- ENB 998/mentorship preparation or equivalent.
- 6-steps Insulin Management training (desirable).
- Experience working in a community setting (desirable).
