About the Role

We are seeking a highly motivated and experienced Peripatetic Insulin Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our community nursing team, you will be responsible for providing high-quality insulin care to patients in their own homes, care homes, and clinics.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care, developing individualized care plans that meet the unique needs of each patient.
  • Work collaboratively with GPs, Social Services, and other healthcare professionals to ensure seamless care coordination and patient outcomes.
  • Provide education and support to patients and their families on insulin management and diabetes care.
  • Conduct holistic assessments, communicating complex information to patients and carers, and adapting explanations to meet individual needs.
  • Liaise with multidisciplinary teams, including Triage and Diabetes Specialist Teams, to ensure effective patient care and discharge planning.
  • Participate in multi-agency meetings, such as the Gold Standard Framework, to ensure patient-centered care.
  • Ensure accurate and timely documentation of patient records and SystmOne inputting.
  • Contribute to the requisitioning of supplies and equipment through electronic ordering systems.
  • Adhere to infection control policies and universal precautions when exposed to bodily fluids and infected materials.
Person Specification
  • Essential:
    • Full and valid driving license and access to a vehicle.
    • Diploma or 1st-level degree in nursing.
    • Current NMC registration.
    • ENB 998/mentorship preparation or equivalent.
    • Experience of working within a clinical setting.
  • Desirable:
    • 6-steps Insulin Management training.
    • Experience of working in a community setting.
