About the Role

We are seeking a highly motivated and experienced Peripatetic Insulin Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our community nursing team, you will be responsible for providing high-quality care to patients with diabetes, working closely with GPs, Social Services, and other healthcare professionals to ensure seamless care coordination.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care, developing individualized care plans that meet the unique needs of each patient.
  • Work collaboratively with the multidisciplinary team to facilitate early discharge from hospital, prevent unnecessary admissions, and ensure timely and effective care.
  • Provide education and support to patients and their families, promoting self-management and independence in diabetes care.
  • Conduct holistic assessments, communicating complex information to patients and carers, and adapting communication styles to suit individual needs.
  • Liaise with healthcare professionals, statutory agencies, and the voluntary sector to ensure identified needs are met and care is coordinated effectively.
  • Participate in multidisciplinary meetings, contributing to the development of care plans and ensuring seamless care coordination.
  • Ensure accurate and timely documentation of patient records, using electronic systems to input data and maintain up-to-date records.
  • Contribute to the requisitioning of supplies and equipment, utilizing electronic ordering systems to ensure efficient and effective resource management.
Person Specification
  • Essential:
    • Full and valid driving license and access to a vehicle.
    • Diploma or 1st-level degree in nursing.
    • Current NMC registration.
    • ENB 998/mentorship preparation or equivalent.
    • Experience of working within a clinical setting.
  • Desirable:
    • 6-steps Insulin Management training.
    • Experience of working in a community setting.
