Peripatetic Insulin Nurse Specialist

We are seeking a highly motivated and experienced Peripatetic Insulin Nurse to join our team at Norfolk Community Health and Care NHS Trust. The successful candidate will be responsible for providing high-quality care to patients with diabetes, working closely with our Unplanned Care Response Team and other healthcare professionals to ensure seamless care delivery.
Main Responsibilities- Assess, plan, implement, and evaluate patient care, developing individualized care plans using clinical reasoning skills.
- Devise evidence-based care programs for patients, modifying them as needed.
- Communicate complex information to patients and carers, adapting explanations to meet their needs.
- Liaise with GPs, Social Services, and other healthcare professionals to ensure identified needs are met and care is coordinated.
- Participate in multi-disciplinary meetings, contributing to the requisitioning of supplies and equipment.
- Ensure patient-held records are completed and SystmOne inputting is done daily.
- Use tact and diplomacy when communicating with patients and carers.

Requirements- Must hold a full and valid driving licence and have access to a vehicle.
- Diploma or 1st-level degree in a relevant field.
- Current NMC registration.
- ENB 998/mentorship preparation or equivalent.
- Experience of working within a clinical setting.
- 6-steps Insulin Management training.
- Experience of working in a community setting.
