Job Overview

We are seeking a skilled and compassionate Registered Nurse to join our Community Continence and Urology Service team. As a Clinical Continence Specialist, you will play a vital role in providing specialist care to patients, their families, and carers across the West Cheshire Footprint.

Key responsibilities will include conducting continence assessments, implementing and evaluating treatment programmes, and participating in nurse-led continence clinics. You will also be responsible for maintaining accurate and confidential caseload notes, communicating effectively with relatives, clients, and the multi-disciplinary team, and advocating for patients' needs.

The successful candidate will have a strong background in community nursing, with experience in multi-agency working and a working knowledge of local and national priorities. You will be able to demonstrate high standards of nursing practice, manage time effectively, and possess good interpersonal skills.

Main Duties of the Job
  1. To carry out continence assessments, implement and evaluate treatment programmes to patients.
  2. Professionally responsible for own caseload.
  3. Participate in nurse-led continence clinics.
  4. Perform clinical skills relating to the post.
  5. Training to carers.
  6. Responsible for maintaining own confidential caseload notes.
  7. Work with external agencies to raise profile of continence promotion.
  8. To communicate effectively with relatives, clients, agencies and the multi-disciplinary team.
  9. To act as the clients advocate at all time
  10. To develop communication strategies to ensure understanding of care programmes and concordance with the treatment plan.
  11. To encourage good relationships with other statutory and voluntary services involving collaborative working.
  12. To have good communication within the continence team.
  13. To effectively communicate sensitive information in relation to, e.g., financial, sexual, interpersonal issues.
  14. Responsible for managing and prioritising own caseload.
  15. To work within the continence service budget.
  16. Participate in risk assessment for patients, carers and staff.
  17. Signpost and advise staff as to sources of information as appropriate to their needs/requests.
  18. Participate in the audit process in conjunction with the clinical audit department where appropriate.
