Job Overview

We are seeking a highly skilled and dedicated Registered General Nurse to join our Community Continence and Urology Service on a full-time permanent basis. As a key member of our team, you will be responsible for providing specialist care to patients, their families, and carers across the West Cheshire Footprint.

The successful candidate will have a strong background in community nursing and multi-disciplinary working, with a passion for delivering high-quality care to individuals with continence and urology needs. You will be expected to work effectively with external agencies, raise the profile of continence promotion, and communicate effectively with relatives, clients, and the multi-disciplinary team.

Main Duties of the Job
  1. Carry out continence assessments, implement, and evaluate treatment programmes to patients.
  2. Professionally responsible for own caseload.
  3. Participate in nurse-led continence clinics.
  4. Perform clinical skills relating to the post.
  5. Train carers.
  6. Responsible for maintaining own confidential caseload notes.
  7. Work with external agencies to raise the profile of continence promotion.
  8. Communicate effectively with relatives, clients, agencies, and the multi-disciplinary team.
  9. Act as the client's advocate at all times.
  10. Develop communication strategies to ensure understanding of care programmes and concordance with the treatment plan.
  11. Encourage good relationships with other statutory and voluntary services involving collaborative working.
  12. Have good communication within the continence team.
  13. Effectively communicate sensitive information in relation to, e.g., financial, sexual, interpersonal issues.
  14. Responsible for managing and prioritizing own caseload.
  15. Work within the continence service budget.
  16. Participate in risk assessment for patients, carers, and staff.
  17. Signpost and advise staff as to sources of information as appropriate to their needs/requests.
  18. Participate in the audit process in conjunction with the clinical audit department where appropriate.
