About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Home Treatment Team based in Congleton. As a key member of our team, you will be responsible for providing high-quality care and support to individuals with mental health needs.

Key Responsibilities
  • Work across Urgent Care Mental Health First Response Services, including Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage Services, Liaison and Diversion Services, and the Crisis Triage and Response Service.
  • Provide expert care and support to individuals with mental health needs, working closely with other healthcare professionals to ensure comprehensive care.
  • Develop and implement effective treatment plans, taking into account the individual's unique needs and circumstances.
  • Collaborate with other teams and services to ensure seamless care and support for individuals.

Person Specification

We are looking for a Mental Health Practitioner with a recognised qualification in mental health, such as RMN, AHP, Psychology, or Social Care. You will also have evidence of further professional development involving serious mental illness, and a non-medical prescribing certificate is desirable.

You will have sound knowledge of current professional staff development and continuous service improvement issues, particularly in the context of ADHD and personality disorder services. You will also have knowledge and sensitivity to equal opportunities issues.
