Mental Health Practitioner

We are seeking a highly skilled and dedicated Registered Nurse to join our Community Continence and Urology Service on a full-time permanent basis. As a key member of our team, you will be responsible for providing specialist care to patients, their families, and carers across the West Cheshire Footprint.
The successful candidate will have a strong interest in continence and urology, with excellent communication and interpersonal skills. You will work closely with our established team of clinicians, nurses, and administrators to deliver high-quality care and support to our patients.
Main Duties of the Job- Conduct comprehensive continence assessments and develop individualized treatment plans.
- Implement and evaluate specialist continence programs of care, ensuring effective management of patient needs.
- Participate in nurse-led continence clinics, providing expert advice and guidance to patients and carers.
- Develop and maintain strong relationships with external agencies, promoting continence awareness and education.
- Communicate effectively with patients, families, and carers, ensuring their needs are met and their expectations are exceeded.
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery.
- Contribute to the development and implementation of continence policies and procedures.
- Participate in quality improvement initiatives, ensuring continuous improvement in patient care and outcomes.

Person SpecificationThe successful candidate will possess:
- Registered General Nurse part 1 of the NMC register.
