We are seeking a highly skilled and experienced District Nurse Caseload Manager to join our Unplanned Care Team. As a key member of our team, you will be responsible for leading a team of qualified nurses, nursing associates, and healthcare support workers in providing high-quality care to patients in their own homes.
The successful candidate will have excellent clinical skills, leadership, and management qualities, with the ability to implement high standards of patient care and ensure a positive patient experience. You will be working in a fast-paced environment, responding quickly to patients' needs and working collaboratively with other healthcare professionals to achieve the best outcomes.
Main Duties of the Job
The District Nurse Caseload Manager will be responsible for the day-to-day smooth running of the caseload, holding clinical responsibility for the patients they are managing and leading on. This will involve ensuring patients are appropriately assessed and care is delivered in a timely manner, with the involvement of patients and carers as appropriate.
You will also be responsible for ensuring team members are appropriately trained, supported, and have the right knowledge, skills, and competencies to meet the requirements of the role.

Person Specification
The successful candidate will have current NMC registration, experience of care co-ordination and care management, and an awareness of current health and social care policies. A qualification in the management of one or more chronic disease or LTC would be desirable.
