About the Role
We are seeking a highly motivated and experienced registered nurse to join our Elderly Virtual Ward team as a Clinical Team Lead. As a key member of our multidisciplinary team, you will be responsible for leading and managing a clinical team, delegating work to staff, and supporting the development of nurse-led initiatives.
Main Responsibilities
- Lead and manage a clinical team, ensuring high-quality patient care and efficient service delivery.
- Develop and implement seamless and integrated care pathways between acute wards and primary care.
- Support the development of nurse-led initiatives and collaborate with clinicians in partner organizations.
- Act as a role model for staff within the EVW/FDF service and provide supervisory responsibility for Nursing/therapy, support, and administration staff.
About Us
As a member of our team, you will be part of a friendly, supportive, and professional integrated team that comprises Geriatricians, Nurses, Occupational Therapists, Physiotherapists, Co-ordinators, Advanced Clinical Practitioners, Pharmacists, Social services, and Community services.
Requirements
- Registered Nurse with a 1st-level degree and current NMC registration.
- Extensive post-registration experience in relevant clinical specialty - Frailty, with significant knowledge and skills in Frailty.
- Excellent communication and organisational skills, with the ability to communicate in complex situations to all levels of staff and service users.
