The Cancer Tracker/MDM Coordinator will work closely with clinical and non-clinical staff in our cancer multidisciplinary teams, providing essential support during periods of annual leave or sickness absence. Key responsibilities include:
- Collaborating with multidisciplinary team members and clerical staff to ensure timely movement and tracking of cancer patients through their journey.
- Identifying and addressing barriers to individual patient pathways and potential breaches of waiting time guarantees, escalating concerns to the appropriate manager as needed.
- Facilitating the collection, analysis, and presentation of audit data to inform patient care and monitor key performance indicators.
- Coordinating regular multidisciplinary team meetings to ensure a smooth patient journey and timely management of cancer cases.
- Providing administrative support to the Specialist Nurse team.
