About the Role:

We are seeking an experienced Nurse Case Manager to join our team at Appoint Healthcare. This is a challenging and rewarding role that will see you working closely with nurses and care support workers to ensure the delivery of high-quality care.

Key Responsibilities:

  • Managing a caseload of nurses and care support workers, ensuring the delivery of high-quality care;
  • Working closely with the office team to plan workloads and duties efficiently, supporting the day-to-day operations and handling administrative tasks;
  • Supporting the development of registered nurses and care workers, providing clinical training, supervision, and competency sign-offs;
  • Being involved in the full care planning process – assessment, planning, implementation, and evaluation;
  • Attending review meetings, adult/child protection meetings, and coordinating with care managers, social services managers, and primary care professionals;

Requirements:

  • NMC PIN – Registered Children's Nurse or RGN with paediatric experience;
