About the Role:

We are seeking a highly skilled and dedicated individual to join our team as a Stroke Recovery Service Coordinator. As a key member of our team, you will play a vital role in supporting stroke survivors and their carers to achieve their goals and maximize their recovery.

Key Responsibilities:

  • Supporting new stroke survivors and their carers from hospital discharge into the community.
  • Providing personalized information, advice, and support to enable stroke survivors to make informed lifestyle changes.
  • Working with other health and social care professionals across the stroke pathway to ensure high-quality support.

Requirements:

  • Experience in a caring profession, ideally supporting people with disabilities.
  • Excellent IT skills and ability to maintain accurate records.
  • Affinity with the values of the Stroke Association.
  • Flexible approach and ability to effectively manage a caseload.

Travel Requirements:

This role requires frequent travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.

Residency Requirements:

To fulfill the role, you must be a resident of the UK and have the right to work in the UK.
