About the Role

We are seeking an innovative and passionate individual to join our team as a Support Coordinator for our Stroke Recovery Service in Norfolk and Waveney. As a result of increased funding to support stroke services, we are looking to recruit five new support coordinators to help us deliver our strategy and make a real difference.

Key Responsibilities

  • Support new stroke survivors and their carers from hospital discharge into the community.
  • Provide personalised information, advice and support.
  • Enable stroke survivors to make informed lifestyle changes to prevent further strokes.
  • Work with other health and social care professionals across the stroke pathway to ensure high-quality support.

Requirements

  • Experience in a caring profession, ideally supporting people with disabilities.
  • Excellent IT skills and ability to maintain accurate records.
  • Affinity with the values of the Stroke Association.
  • Flexible approach and ability to effectively manage a caseload.

Travel Requirements

This role requires frequent travel across a large geographical locality to visit people at home and in community settings across the Norfolk and Waverley area. Candidates must be able to demonstrate how they can meet this requirement of the role.

Residency and Work Requirements

To fulfill the role, you must be a resident of the UK and have the right to work in the UK.
