Job Title: Stroke Review Coordinator

We are seeking a highly organized and detail-oriented individual to join our team as a Stroke Review Coordinator. This is a fantastic opportunity to work in a supportive multidisciplinary rehabilitation team, assisting with the administration and clerical duties for the Community Stroke Team.

About the Role:

The successful candidate will be responsible for completing six-month stroke reviews, either virtually or in patients' homes, as outlined in the National Stroke guidelines. They will also assist with data base management, data collection, processing of referrals, and answering phone queries from stakeholders.

Key Responsibilities:
  • Complete six-month stroke reviews, including planning home visits, undertaking blood pressure, and maintaining databases.
  • Participate in multidisciplinary / multi-agency team meetings and case conferences to ensure coordination of patient care.
  • Maintain accurate clinical records and update databases relating to the six-month stroke reviews.
  • Input data into databases, including the SSNAP database.
Requirements:
  • Evidence of study to or attainment on NVQ level 4; or equivalent knowledge and experience.
  • Qualification in Business Administration.
  • Certification in Microsoft Office.
