We are seeking a highly motivated and experienced Support Coordinator to join our team in delivering our Stroke Recovery Service in Norfolk and Waverley. As a key member of our team, you will play a vital role in supporting stroke survivors to achieve their goals and maximize their recovery.
Key responsibilities will include:
- Providing personalized support and guidance to stroke survivors and their carers.
- Working closely with healthcare professionals to ensure seamless support and care.
- Developing and implementing individualized recovery plans to promote independence and well-being.
- Collaborating with other teams to ensure effective communication and coordination of care.
You will have:
- Experience in a caring profession, ideally supporting people with disabilities.
- Excellent communication and interpersonal skills.
- A flexible approach and ability to manage a caseload effectively.
- An affinity with the values of the Stroke Association.
This role requires frequent travel across the Norfolk and Waverley area, so you must be able to demonstrate how you can meet this requirement.
