About the Role

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our team at Sussex Partnership NHS Foundation Trust. As a Community Mental Health Nurse - Band 6, you will work in partnership with service users, families, and carers to deliver high-quality care and support in the community.

Main Responsibilities

  • Undertake assessments of individuals and their families with complex health presentations, including those presenting with higher levels of risk.
  • Develop and implement care plans and recovery plans to enable service users to lead fulfilling lives.
  • Work collaboratively with carers, families, and partner agencies to facilitate care and support.
  • Provide specialist nursing advice and carry out specialist nursing procedures.

Requirements

  • Registered Nurse with current registration in mental health or a related field.
  • Significant post-qualifying experience in mental health nursing.
  • Experience of working with people with complex health problems and their families.
  • Ability to work autonomously and as part of a multidisciplinary team.
