About the Role
We are seeking a skilled Mental Health Nurse to join our team at Sussex Partnership NHS Foundation Trust, working within our Intensive Support Service supporting individuals with dementia and mental health needs in the community.
The successful candidate will work alongside our multidisciplinary team, delivering interventions to those with complex or intensive needs. Our goal is to enable individuals to remain safe and well at home, maintaining meaningful relationships.
Main Responsibilities
This role involves assessing, planning, and implementing care for service users with complex health presentations. The post holder will develop individualized care plans, ensuring evidence-based practice consistent with NICE guidelines and Trust policies. They will provide supervision to junior staff, act as an autonomous practitioner, and demonstrate expertise in clinical interventions relevant to the designated care group.
Person Specification
To be successful in this role, you will require:
- A Registered Nurse qualification with current registration in mental health or adult nursing.
- Completion of a mentorship course or ENB equivalent.
- Evidence of continuous professional development.
- Significant post-qualifying experience in the relevant care group.
- Experience working alongside individuals with complex health problems.
- Experience supervising staff, conducting clinical assessments, and partnership working.
