About the Service

Southwark Council's Hospital Discharge Transfers of Care Unit is dedicated to providing seamless care transitions from hospital to home or other care settings. Our team is responsible for managing hospital discharges, safeguarding concerns, and care planning with precision and compassion.

About the Role

In this pivotal role, you'll lead and inspire your team to deliver high-quality care every day. Key responsibilities include:

  • Operational Leadership: Oversee the day-to-day operations from Friday to Tuesday, ensuring social workers efficiently triage referrals, allocate cases, and authorise assessments.
  • Guidance and Support: Provide expert guidance on complex discharges, safeguarding issues, and mental capacity assessments, ensuring your team is well-supported and empowered to make informed decisions.
  • Collaborative Networking: Serve as the lead manager for weekend hospital discharges at Kings and St Thomas Hospital, ensuring that care plans are maintained, and discharges are handled effectively.
  • Team Development: Mentor and supervise social workers and assistant practitioners, fostering an environment of continuous learning and reflective practice.
About You

We are looking for a dynamic, experienced leader with a strong commitment to social care excellence. A solid understanding of the Care Act, Mental Capacity Act, and Continuing Healthcare processes is essential.

Proven experience in leading MDT and safeguarding meetings, providing reflective supervision, and supporting the professional development of others is highly desirable.
