About the Role

We are seeking an experienced leader to oversee the weekend operations of our Hospital Discharge Transfers of Care Unit. As a Weekend Care Unit Leader, you will be responsible for ensuring the efficient triage of referrals, allocation of cases, and authorisation of assessments from Friday to Tuesday.

Our ideal candidate has a strong understanding of the Care Act, Mental Capacity Act, and Continuing Healthcare processes, with experience in leading MDT and safeguarding meetings, providing reflective supervision, and supporting the professional development of others.
