Job Title: Team Sister/Charge Nurse

An exciting opportunity has arisen for a skilled and dedicated registered nurse to join our East Lancashire District Nursing Team as a Team Sister/Charge Nurse.

About the Role

We are seeking a highly motivated and experienced nurse to lead our team in delivering high-quality patient care in the community. As a Team Sister/Charge Nurse, you will be responsible for coordinating and delivering care in line with District Nursing Strategy nationally and locally.

Main Responsibilities
  • Assist the Clinical Team Leader in the coordination and delivery of care
  • Assess new and complex patients
  • Deliver comprehensive Senior Nurse Reviews of patients in their own homes and other community care settings
  • Work as part of a wider multidisciplinary team
  • Support students and new staff in learning and development
  • Develop the team through leadership and role modeling
  • Manage staffing rotas and allocate clinical work
  • Establish and maintain effective communication with all clinical staff, patients, and families
Requirements
  • Substantial district nursing experience
  • Experience in holistic assessment of complex patients and care coordination
  • Experience of multidisciplinary team working
