Community Nurse

We are seeking a highly skilled and dedicated Community Nurse to join our District Team at East Lancashire Hospitals NHS Trust.
Key Responsibilities- Assist the Clinical Team Leader/Team Sister in the coordination and delivery of care in line with District Nursing Strategy nationally and locally.
- Visit patients in their own homes and provide high-quality nursing care to a range of patients with varying healthcare needs.
- Provide holistic assessments and care, including wound management, health promotion, medicines management, and end-of-life care.
- Work contracted hours rotationally across day and night shifts, as reasonably required by the needs of the service.
- Professional qualification in RGN.
- Experience of multi-disciplinary working and community working.
- Good communication skills and ability to adapt to the needs of the service and work independently.
- Clinical knowledge and skills, including knowledge of professional and NHS issues.
