An exciting opportunity has arisen for a skilled and dedicated registered nurse to join the East Lancashire District Nursing Team as a Team Sister. The successful candidate will have the ability to work contracted hours rotationally across day and night shifts, as reasonably required by the needs of the service. This is a 7-day service, including weekends and Bank Holidays.
Main Duties of the Job- Assists the Clinical Team Leader in the coordination and delivery of care in line with District Nursing Strategy nationally and locally.
- Assessment of new and complex patients.
- Delivery of comprehensive Senior Nurse Reviews of patients in their own homes and other community care settings.
- Working as part of a wider multidisciplinary team.
- Supporting students and new staff in learning and development in the service.
- Development of the team through leadership and role modeling.
- Management of staffing rotas and allocation of clinical work.
- Establishing and maintaining effective communication with all clinical staff, patients, and families.
- Working in the District Nursing Hub to provide weekend, evening, and bank holiday cover (on a rota basis).
- Registered Nurse (i.e., RN Adult branch).
- Community Specialist Practitioner Qualification or willingness to work towards this.
- Non-medical prescriber V100/V150/V300.
- Leadership training.
- Substantial district nursing experience (12 months).
- Experience in holistic assessment of complex patients and care coordination.
- Experience of multidisciplinary team working.
- Ability to develop effective working relations with other agencies and professionals.
- Ability to manage own and others' workload and delegate effectively.
The post holder must demonstrate professional leadership and be passionate about patient care. They will need to be able to problem-solve and adapt to challenging situations, manage resources efficiently, and ensure the delivery of safe, personal, and effective patient care. We are looking for a nurse with a commitment to quality care and the ability to positively challenge to ensure patient and staff safety.
Car driver with access to a vehicle is essential.
