Job Overview

We are seeking an enthusiastic and experienced Community Staff Nurse to join our team as a Deputy Team Leader in the Community Nursing Team. This is an exciting opportunity to enhance and develop your leadership knowledge and skills in a dynamic role.

Main Responsibilities

  • Lead and manage the caseload of the Community Nursing Team, monitoring standards of care, delegating responsibilities, and supervising team members as appropriate.
  • Develop and maintain effective communication systems, working closely with patients, carers, and families, and other members of the integrated care team and statutory and voluntary agencies.
  • Act as care manager for identified patients to prevent inappropriate hospital admission, working in partnership with other agencies.
  • Provide care planning, support, treatment, and monitoring to patients on the general caseload, in collaboration with the wider integrated care team.
  • Manage resources effectively and advise on skill mix requirements for the team, in collaboration with the Team Leader.
  • Assist with the management of team budgets, in collaboration with the Team Leader.
  • Participate in and promote Clinical Governance, ensuring the highest quality of practice is maintained, within the service. Demonstrate the use of evidence-based practice and participation in clinical audit.
  • Assist with performance reviews within the team to contribute to team and corporate objectives, encouraging and supporting development of staff and maintenance of personal development plans.
  • Ensure team members undertake mandatory training requirements.

Requirements

  • Registered Nurse Level 1
  • District Nurse degree or equivalent (or a willingness to complete DN Qualification as directed by management)
  • Experience of administration of Intravenous medications (or a willingness to complete training within an agreed timescale)
  • Completed the national PEAP programme for practice supervisors or equivalent teaching and assessing qualification for those with ENB998

Desirable Criteria

  • Community Practitioner Nurse Prescriber / Independent Prescriber
  • Masters level study in long term conditions

Experience

  • Experience of working with older people, palliative care, and chronic disease
  • Continuing professional knowledge and development
  • Experience of teaching and mentorship
  • Evidence of knowledge and experience of Case Management
  • Change management and leadership skills
  • Working in partnership with other healthcare providers, Social services, etc.
  • Experience in the management of staff
  • Experience of multidisciplinary working

Skills and Knowledge

  • Sound understanding of Primary care and multi-agency working
  • Awareness of the value of audit/research and evidence-based practice
  • Awareness of NHS issues
  • IT skills
  • Audit skills
  • Evidence of ability to communicate both verbally and in writing at all levels in multi-disciplinary forums and with users and carers.
  • Able to organise and coordinate services in collaboration with, and in the absence of the District Nurse Team Leader

Personal Qualities

  • Due to the requirement to undertake visits and transport non-portable equipment, the post holder is required to be either a car driver with a valid driving licence, or have access to an alternative means of transport.
