Nurse Advisor for Primary Care

About the Opportunity: We are seeking a highly skilled Nurse Advisor to join our team at Chemistree Solutions Ltd. As a Nurse Advisor for Primary Care, you will play a vital role in delivering exceptional patient care and improving patient outcomes in the field of Diabetes.
Key Responsibilities:
- Working in partnership with the NHS in primary care to deliver a unique nursing programme
- Collaborating with patients and practices to drive best practice and deliver excellent patient care
- Assisting healthcare professionals in delivering up-to-date care for their patients
Requirements:
- A minimum of 3 years' experience (post-registration) managing diabetes within primary care
- Knowledgeable and confident in the provision of education, support, and advice to people with Type 2 Diabetes
- Skills and knowledge in the optimisation of oral hypoglycaemics
- IT literacy, including primary care systems
Compensation and Benefits:
- A competitive basic salary: £35,000 - £50,000 per annum
- A company car or car allowance
