We are seeking a highly skilled Senior Community Physiotherapist/Occupational Therapist to join our dedicated Neuro Rehabilitation Team in South Yorkshire. As a Band 6 Community Physiotherapist/Occupational Therapist, you will be responsible for delivering comprehensive rehabilitation services to individuals within their own homes and community settings.

Key Responsibilities:

  • Develop and implement individualised treatment plans, including therapeutic exercises, mobility training, and adaptive equipment prescription.
  • Collaborate closely with other healthcare professionals, including nurses, social workers, and community support workers, to ensure coordinated care delivery.
  • Maintain accurate and up-to-date clinical records in accordance with organisational policies and professional standards.

Requirements:

  • Degree in Physiotherapy or Occupational Therapy from a recognised institution.
  • Registration with the Health and Care Professions Council (HCPC).
  • Minimum of two years' experience working as a Physiotherapist or Occupational Therapist in a community setting.
  • Strong clinical assessment and treatment planning skills.
  • Commitment to continuous professional development and evidence-based practice.
  • Valid and Clear Driving Licence.
