About the Role:
As a Community Healthcare Specialist, you will be responsible for delivering exceptional nursing care to patients in their own homes and community settings. You will work closely with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
• Assess, plan, implement, and evaluate individualized care plans for patients with complex needs.
• Administer medications, wound care, injections, and other nursing procedures as required.
• Provide emotional support and guidance to patients and their families, promoting health education and disease prevention.
• Collaborate with healthcare professionals to ensure holistic patient care, including GPs, social workers, and other specialists.
• Maintain accurate patient records and report on patient progress.
• Identify and mitigate risks, adhering to NHS guidelines and best practices.

Requirements:
• Registered Nurse (RN) with a valid NMC Pin.
• Experience in community healthcare or district nursing preferred but not essential.
• Strong clinical skills and ability to work independently and as part of a team.
• Excellent communication and interpersonal skills, with a focus on building rapport with patients from diverse backgrounds.
• Knowledge of health and safety regulations and infection control practices.

Benefits:
• Salary range: £40,000 - £55,000 per annum.
• Flexible working arrangements, including 12-hour shifts, 8-hour shifts, day shifts, night shifts, and overtime opportunities.
