Healthcare Provider Coordinator

We are seeking a compassionate Healthcare Provider Coordinator to join our team at Nurslink. As a key member of our multidisciplinary team, you will work closely with patients, families, and healthcare professionals to deliver high-quality patient care.
Key Responsibilities- Assess and plan individualized care for patients with complex health needs.
- Provide expert nursing care to patients in their own homes and community settings.
- Collaborate with healthcare professionals to ensure holistic patient care.
- Maintain accurate patient records and report on patient progress.
Qualifications and skills required for this role include a Registered Nurse (RN) qualification, experience in community healthcare or district nursing, and knowledge of health and safety regulations and infection control practices. Salary: £38,000 - £50,000 per annum, based on experience and location.
