Healthcare Professional for Community Care

Nurslink is seeking a skilled and compassionate Registered Nurse to provide high-quality nursing care to patients in their homes, residential care settings, or community clinics in Cumbria.
About the Role
As a dedicated member of our healthcare team, you will manage a diverse caseload of patients with various health needs. You will work closely with multidisciplinary teams to promote health, independence, and quality of life for our patients.
Key Responsibilities:
- Provide individualized care plans for patients with complex needs.
- Administer medications, wound care, injections, and other nursing procedures as needed.
- Offer support and guidance to patients and their families, promoting health education and disease prevention.
- Collaborate with GPs, social workers, and other healthcare professionals to ensure holistic patient care.
- Maintain accurate patient records and report on patient progress.
- Conduct risk assessments and ensure that care is delivered in accordance with NHS guidelines and best practices.
- Act as an advocate for patients, ensuring their voice is heard in decision-making.
- Participate in multidisciplinary team meetings and case conferences.
- Support the discharge process by coordinating care for patients transitioning from hospital to home.
Qualifications and Skills:
- Registered Nurse (RN) with a valid NMC Pin.
- Experience in community healthcare or district nursing preferred but not essential.
- Strong clinical skills, with the ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Compassionate, empathetic, and able to build rapport with patients from diverse backgrounds.
- Knowledge of health and safety regulations and infection control practices.
Salary and Benefits:
The salary for this role is £25,000 - £35,000 per annum, depending on experience. This includes a comprehensive benefits package, including a referral programme.
Work Schedule:
- 12 hour shifts and 8 hour shifts available.
- Monday to Friday schedule with flexible shift options.
Licence/Certification:
- NMC registration required.
Location:
This role is based in Cumbria, working in patients' homes, residential care settings, or community clinics.
