We are seeking a Clinical Care Specialist to provide high-quality nursing care in the Cumbria community. The successful candidate will work closely with multidisciplinary teams to promote health, independence, and quality of life.



The role involves managing a diverse caseload of patients with a variety of health needs. Key responsibilities include:




  • Providing essential nursing care to individuals in their homes, residential care settings, or community clinics
  • Assessing, planning, implementing, and evaluating individualized care plans for patients with complex needs
  • Administering medications, wound care, injections, and other nursing procedures as needed
  • Offering support and guidance to patients and their families, promoting health education and disease prevention
  • Collaborating with GPs, social workers, and other healthcare professionals to ensure holistic patient care
  • Conducting risk assessments and ensuring that care is delivered in accordance with NHS guidelines and best practices
  • Supporting the discharge process by coordinating care for patients transitioning from hospital to home
  • Providing end-of-life care, ensuring patient comfort and dignity


The ideal candidate will have a valid NMC Pin and experience in community healthcare or district nursing (preferred but not essential). Knowledge of health and safety regulations and infection control practices is essential.
