Physiotherapist - Community Health Specialist

We are seeking a highly skilled and experienced Physiotherapist to join our clinical team at Central London Community Health Trust. As a Community Physiotherapist, you will play a vital role in providing a comprehensive Physiotherapy service to residents of Brent.
Key Responsibilities- Be an active member of the clinical team, working collaboratively with other healthcare professionals to provide a seamless service.
- Provide a comprehensive Physiotherapy service to residents of Brent, including assessments, treatment plans, and ongoing care.
- Work within professional standards and clinical guidelines, promoting best practice and undertaking all aspects of clinical duties as an autonomous practitioner.
- Supervise junior staff, rehabilitation assistants, and physiotherapy students, providing guidance and support to ensure high-quality care.
- Work in a variety of settings, including clients' own homes, clinics, residential and care homes, community facilities, continuing care homes, and community hospitals.
- Support self and service development through active participation in Continuing Professional Development (CPD) and by participating in any audit or research being undertaken by the service.
- Undertake specialised uni-professional and multi-professional assessments, including complex specialist assessments as appropriate.
- Develop and implement treatment plans for clients referred to the different services, taking into account their complex multipathologies and specialist areas.
- Participate in the Single Assessment Process when appropriate, performing overview assessments and complex specialist assessments as necessary.
- Be responsible for gaining valid, informed consent for assessment, treatment interventions, and sharing of information in compliance with legal and professional guidelines.
- Have working knowledge of clinical guidelines, specific to the area you are working in, and translate these guidelines into clinical practice with the guidance of your supervisor.
We provide community health services to more than two million people across eleven London boroughs and Hertfordshire. Our professionals provide high-quality healthcare in people's homes and local clinics, helping them to stay well, manage their own health with the right support, and avoid unnecessary trips to or long stays in hospital.
We support our patients at every stage of their lives, providing health visiting for new-born babies through to community nursing, stroke rehabilitation, and palliative care for people towards the end of their lives.
Job Requirements- Essential qualifications: Degree or diploma in physiotherapy, registered with the Health Care Professions Council, and hold relevant right to work in the UK documentation.
- Desirable qualifications: Membership of Chartered Society of Physiotherapy and/or relevant special interest groups.
- Essential experience: Appropriate post-qualifying experience in a range of areas, including acute care of the elderly, neurology, and community. Ability to assess patient conditions, establish the patient's problem, formulate and implement a treatment plan.
- Desirable experience: Previous community experience within rehab, experience of using an electronic clinical records system.
- Essential skills: Advanced skills in the assessment and treatment of people with complex multi-pathologies, advanced knowledge and understanding of complex medical conditions and their relation to people who fall, knowledge and understanding of National and local frameworks and guidelines for older people.
- Desirable skills: Highly developed specialist knowledge and application of physiotherapy assessments and interventions relevant to client group.
- Demonstrate physical ability/fitness to carry out occupational assessment and intervention with appropriate manual handling +/- equipment.
- Ability to transport light pieces of equipment from peripheral stores and office bases to patient homes via your desired means of transport.
- Demonstrate physical ability to be able to carry a standard backpack with medical supplies/laptop required to fulfil essential clinical duties.
