About the Role:

We are seeking a highly organized and compassionate individual to join our team in Tunbridge as a Care Coordinator. As a key member of our care team, you will be responsible for coordinating care plans and ensuring the delivery of high-quality services to clients. This role involves occasional call-out responsibilities to address urgent needs.

Key Responsibilities:

  • Develop and manage care schedules to ensure efficient service delivery.
  • Liaise with clients, families, and care staff to create and review care plans.
  • Conduct assessments and ensure care meets regulatory standards.
  • Monitor and address any issues arising from care provision.
  • Handle urgent call-outs to provide support to clients or resolve issues as they arise.
  • Maintain accurate records and ensure compliance with company policies and regulations.
  • Work collaboratively with team members and external agencies to achieve positive outcomes.

Requirements:

  • Proven experience in care coordination or a similar role.
  • Strong organizational and communication skills.
  • Ability to respond quickly and effectively during call-outs.
  • Knowledge of care standards and regulations.
  • Driving license and ability to travel locally for call-outs.
