Care Coordinator Professional

We are seeking a highly organized and compassionate Care Coordinator to join Cpl UK Healthcare in Tunbridge. This role involves coordinating care plans and ensuring the delivery of high-quality services to clients, as well as occasional call-out responsibilities to address urgent needs.
Key Responsibilities:
* Develop and manage care schedules to ensure efficient service delivery.
* Liaise with clients, families, and care staff to create and review care plans.
* Conduct assessments and ensure care meets regulatory standards.
* Monitor and address any issues arising from care provision.
* Handle urgent call-outs to provide support to clients or resolve issues as they arise.

Requirements:
* Proven experience in care coordination or a similar role.
* Strong organizational and communication skills.
* Ability to respond quickly and effectively during call-outs.
* Knowledge of care standards and regulations.
* Driving license and ability to travel locally for call-outs.
