Specialist Community Practitioner Team Lead

NHS is seeking a highly skilled Specialist Community Practitioner Team Lead to join Jean Bishop Frailty Community Services (JB-CFS). This innovative service provides anticipatory patient-tailored care to frail residents of Hull and surrounding areas, including those in care homes.

In this role, you will provide urgent medical care to frail patients acutely unwell in their usual residence, including care homes. As an independent prescriber with advanced practice qualifications, you will be passionate about caring for older and frail patients, driving the development of a high-profile service within the community.

The service operates 365 days a week, with expectations for successful candidates to be included on weekend and bank holiday rotas.

  • Independent non-medical prescriber V300
  • Mentorship/Practice Assessor qualification and evidence of supervision/teaching/team management
Key Responsibilities
  • Provide holistic care to patients in the community
  • Develop and implement clinical or professional change and evaluation practice
  • Collaborate with statutory and voluntary services
  • Use IT and digital solutions in clinical practice
Requirements
  • Current legislation and impact on community services
  • Evidence-based practice
  • Understanding of the virtual ward concept
  • Understanding of proactive caseload management
  • Understanding of audit processes, Key Performance Indicators and Targets, CQUIN collection data, and Quality Monitoring Programmes
  • Integrated Team working
  • Understanding of research methodology and audit
  • Understanding of the principles of restorative practice
