Job Summary

We are seeking a skilled and compassionate Care Coordinator to join our team in the Gravesend area. As a Care Coordinator, you will work closely with our clients and their families to develop personalized care plans, ensuring that their unique needs are met with dignity and respect.

Key Responsibilities

  • Assist individuals with complex care needs in their own homes, providing support with daily living activities, medication management, and personal care.
  • Develop and implement personalized care plans, working closely with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Conduct clinical observations, monitor client progress, and report any changes or concerns to the care team.
  • Provide education and support to clients and their families on complex care skills, such as enteral feeding, oral suctioning, and tracheostomy care.
  • Collaborate with the care team to ensure that clients receive comprehensive and coordinated care, including ventilation, epilepsy management, and other specialized services.

Requirements

  • Minimum 6 months of experience in a complex care setting or a related field.
  • Strong clinical skills and knowledge of complex care practices, including enteral feeding, oral suctioning, and tracheostomy care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality care and promoting client independence.
