Job Summary

We are seeking a skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will work closely with our clients and their families to develop personalized care plans, ensuring their unique needs are met. Your primary responsibility will be to provide support and guidance to our clients, promoting their independence and well-being.

Key Responsibilities

  • Develop and implement personalized care plans in collaboration with clients, families, and healthcare professionals.
  • Provide emotional support and guidance to clients, promoting their independence and confidence.
  • Assist clients with daily living activities, such as personal care, medication management, and meal preparation.
  • Monitor and report any changes in client condition, ensuring timely interventions and adjustments to care plans.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.

Requirements

  • Minimum 6 months of experience in a care setting, preferably with complex care skills.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and families.
  • Ability to work independently and as part of a team, with a flexible approach to scheduling.
  • Basic knowledge of care principles, including enteral feeding, oral suctioning, and tracheostomy care.
